Chronic Disease Management
Chronic diseases are long lasting health conditions with persistent effects. Their medical, social and economic consequences can have a significant impact on quality of life. As such, optimal management of chronic disease is key to minimising the burden on individuals and their community. The Australian Institute of Health and Welfare has identified the following 10 chronic condition groups as being of most significance in terms of prevalence, impact on daily life, and most importantly that their impact is preventable and modifiable.

Old Bar Medical Centre has a strong focus on high level chronic health care, through our CCPC program. By identifying the above (and other) chronic conditions early, and optimizing their management, future morbidity can be prevented or reduced, thus improving quality of life for patients for years to come.
INTERPRETER SERVICE
We use the Telephone Interpreter Service 1300 131 450 when required. We can organise an “over the phone” service or with 48 hours notice an “on-site” service.
We can arrange interpreters through NABS, National Auslan Inter-preter Service for our deaf patients.
